We want to keep you updated on how we are handling these unprecedented times. In regard to the spread of COVID-19, we want to assure you that we are doing our part to keep our community healthy.
We have made the following changes to our day-to-day operations to ensure we are taking precautions necessary to keep the risk of transmission low.
1. We are asking you to reschedule your appointment at this time if:
– You have experienced an illness within 2 weeks of your appointment.
– You currently have a fever of 100.4*F.
– You have been directly exposed to anyone with a known COVID-19 infection.
– You have traveled outside of the state within 2 weeks of your appointment.
2. ONLY ONE PARENT / GUARDIAN MAY ACCOMPANY THE PATIENT(S) TO ANY TYPE OF APPOINTMENT
– Any additional people (including other children, over the age of 3, that are not scheduled) accompanying the patient to their appointment will be asked to wait outside or in their car to help minimize interaction among individuals.
3. DUE TO HIGH DEMAND AND LIMITED APPOINTMENT AVAILABILITY – IF WE ARE UNABLE TO CONFIRM YOUR APPOINTMENT 2 BUSINESS DAYS IN ADVANCE, IT WILL BE CANCELLED (without penalty).
4. VIRTUAL DENTAL VISITS
We have the ability to conduct a virtual visit for some visit types. Please contact our office if you are interested.
5. ONLINE FORMS
To limit disease vectors, we ask that you take a few minutes and update health information online for each family member scheduled for a dental appointment. This will save you time and streamline your appointment. If you do not complete the required paperwork prior to arriving at the office, your appointment may be delayed.
To complete the patient forms, simply click on the button below, and you will be redirected to our website, select the First Time Use option, and create a web account by using your account id and pin code. Once you have created your web account, you can then use your email address and create a password so that you can easily log-in and review your account at any time.

We know that sometimes things come up. In an effort to maximize our limited PPE supply, and limit waste, we have made the following updates to our scheduling policies:
1. Please arrive on time to your appointments
We do expect some delays during this adjustment period, but you are welcome to contact us prior to your arrival to ensure we are running on time. We apologize for any inconveniences during this period.
2. We kindly request at least 2 business days for cancellations or reschedules to avoid penalty.
– Two (2) No Call / No Shows in one year will result in same-day only appointments or dismissal.
– Two (2) Cancellations, with less than 2 business days notice, in one year will result in same-day only appointments or dismissal.
– Two (2) Reschedules, with less than 2 business days notice, in one year will result in same-day only appointments or dismissal.
WHEN YOU ARRIVE
1. Please stay in your car upon your arrival. You will receive a text message 30 minutes prior to your appointment that will include instructions on how to notify us of your arrival in the parking lot.
2. Patients and parent / guardians will be screened for fever (temperature check) and COVID-19 symptoms / risk factors prior to entering the building.
3. TO ENTER THE FACILITY:
1. ONE (1) PARENT / GUARDIAN CAN ACCOMPANY THE PATIENT(S)
Additional family members will not be allowed inside the facility, including unscheduled children over the age of 3.
2. PARENT / GUARDIAN MUST WEAR A FACE COVERING OR MASK, per CDC guidelines.
4. HYGIENE APPOINTMENTS
We encourage the parent / guardian of any patient 12 years of age or older to wait outside or in their vehicle during their child’s appointment. Special accommodations can be made. Please call prior to appointment to make arrangements.
5. RESTORATIVE APPOINTMENTS
All patients scheduled for RESTORATION OF TEETH, regardless of age, are permitted to have one (1) parent / guardian accompany them into the clinic.
While patient care is our number one priority, the health and well-being of our patients, staff and community are of utmost importance. The following can be done frequently though-out the day to help keep yourself and those around you safe:
1. Wash hands often with soap and water for at least 20 seconds. Use an alcohol-based hand sanitizer that contains at least 60 percent alcohol if soap and water are not available.
2. Avoid touching your eyes, nose, and mouth with unwashed hands.
3. Avoid close contact with people who are sick.
4. Stay home when you are sick.
5. Cover your cough or sneeze with a tissue, then throw the tissue in the trash.
6. Clean and disinfect frequently touched objects and surfaces.
